A 1095-B form confirms your health coverage for tax purposes. Under new regulations, paper copies are no longer required to be automatically mailed. You can request a copy of your 1095-B form be mailed to by contacting the Funds Office by emailing: kimlocal91@yahoo.com or calling phone number: 716-297-2001.
